Transmogrification / Reforging in The Witcher 3
The Remastered reforging system lets Geralt change an item's appearance without replacing its stats.
Short answer. Bring the equipment and the appearance you want to a master craftsman: Yoana or Hattori can reforge armor and swords without changing the equipped item's stats.

Where to find it and what to do
Progress until you can access the master craftsmen. Adding new equipment and crafting diagrams to your inventory unlocks additional appearances.

Details that change the route
Dyes from Blood and Wine carry over to the reforged item, so appearance planning and dyeing can be combined.
Keep old equipment long enough to unlock its appearance before selling it, unless you are certain you will not use that visual.
